E -Consultations Over Zoom
Procedure:
1. Fill up the pre consultation form fully, with your history, symptoms & all medications & email it to drmappointments@gmail.com
The more detailed your descriptions the more you will get out of the consultation.
Send me copies of the pre consultation form & the relevant reports scanned upright.
For your consultation please share all the relevant medical documents which are available with you, including laboratory tests, X-ray films, MRI & CT Scans in digital form. Dicom files of your CT or MRI can be transferred using google drive or wetransfer - a free app. The more information you have available when we have the consultation, the more accurate my diagnosis & the better the prognosis for you.
Please take at least 3 good photos of each iris taken with light shining from below and from each side.
This 2 minute movie can help you to take good photos.
• You will need a good light like a torch, a lamp or a mobile phone LED light, and a smartphone or a digital camera to take clear iris photos.
Use the forward camera on your phone, not the selfie camera side, for higher quality.
• Ask 2 people to help you take the photo. The first person holding the light, first shining it from the side and then from below at a 45 degree angle.
Do not shine the light directly into the eye.
The second person takes the photo zooming in to the iris of the eye
• Take at least two clear Photos of each iris pointing the light from the side and from below.
• The eyes need to be wide open and staring directly into the camera lens.
Good lighting with clear photographs, are required, to give you meaning full advice, along with a properly filled up history.
Your images need to be as large as possible to capture a clear, distortion free view of your eyes.
Please label each photo with your name & left or right iris.

Iris photos need this type of clarity to be used for diagnosis.
Please take the photos with the highest resolution available on your camera so I can enhance them to get the maximum information from them.
2. After looking at the form & photo I will advise you if an an e consultation is possible, with the iris photos & information you have provided.

E Consultations cost Rs 10,000 for a consultation lasting upto 45 minutes.
4. On receipt of the payment I will send you the the invitation link for the meeting on Zoom.
If Zoom does not suit you, we can use FaceTime, Signal, Skype or WhatsApp video.
5. Please set up your camera, test your sound, lighting, etc . 10 minutes before the time of the zoom session so you get the maximum benefit out of your consultation.
Ensure that your lighting is such that I can see you clearly.
6. After the consultation I will send you a note summarising the advice given to you during the call and your receipt for payment.
